OPTION 2: A family who is already signed up refers a friend...
The family who still needs to sign up is the only one who needs to submit the form below.
They will be sent the discount code, and the referring family will be issued a credit to their original payment method. (You don't have to submit this form more than once -- you will automatically be credited when additional friends list you.)
